The transition to integrated care

Population health management in England

Ageing populations and rising patient expectations, together with increasing costs in the face of substantial budget constraints for health and social care, is putting enormous pressure on the sustainability of the NHS. Population Health Management, a patient centric, data driven approach, has been recognised in the NHS Long Term Plan as key to delivering a sustainable health and care system, and improving the lives of the English population.

Population Health brings together an understanding of the health needs of a given population using big-data analytics, patient engagement, and health and care insights. Successful implementation relies on embracing technology and developing models of leadership founded on new ways of working and a shared culture.

This report provides an analysis of how population health can be implemented including:

  • Critical success factors to address in your route-map to implementation
  • Practical frameworks
  • Maturity model mapping the stages
  • Case studies to share and learn from good practice

Listen to our Population Health Webinar

On 15 April 2019, we hosted a webinar to explore population health management.

This webinar was the first of a series of webinars from our Global Population Health Community showcasing and debating global best practice in population health. The webinars will bring together health systems and professionals interested in adopting innovative population health approaches to improve health and wellbeing, providing them with the opportunity to hear first-hand from a pioneer system or organisation.
