Population health management in England

Ageing populations and rising patient expectations, together with increasing costs in the face of substantial budget constraints for health and social care, is putting enormous pressure on the sustainability of the NHS. Population Health Management, a patient centric, data driven approach, has been recognised in the NHS Long Term Plan as key to delivering a sustainable health and care system, and improving the lives of the English population.

Population Health brings together an understanding of the health needs of a given population using big-data analytics, patient engagement, and health and care insights. Successful implementation relies on embracing technology and developing models of leadership founded on new ways of working and a shared culture.
